Hospital-to-outpatient transition and discharge process for stroke patients at tertiary care level hospitals in Peru
Stroke management is complex as it involves different stages in different places. As an example, acute and sub-acute care are provided within the hospital setting, while rehabilitation is provided in the outpatient setting. The transition from hospital to outpatient care can be difficult and patients should be prepared to affront it. One way to do this is to provide them with training and instruction during the discharge process. However, neither the transitions nor the discharge process has been previously studied in Peru. A country where only half of the patients with stroke return to continue stroke management after being discharged. For this reason, we will conduct a qualitative study to better understand this process.
Research Significance
This study will help us understand the hospital-to-outpatient care transition and the discharge process of stroke patients is managed at tertiary care level hospitals in Peru. This will help us identify potential opportunities to improve this processes that could be tested in future studies.
